About the Radiation Planning Assistant

What is the RPA?
The Radiation Planning Assistant provides a suite of fully automated contouring and radiotherapy planning tools designed to deliver consistent, high-quality treatment planning.

The workflow begins with the user uploading a CT scan and completing a Service Request form, after which the planning process is initiated automatically. Patient data are securely transferred, de-identified, and processed within the RPA system to ensure both privacy and reliability.

For complex treatments, such as VMAT planning for head and neck cancers, the system performs automated contouring that is reviewed and edited by the user within their local treatment planning system before being returned to the RPA for final plan generation. For simpler treatments, such as 4-field box plans for cervical cancer, the system can generate a complete plan with minimal user interaction.

In all cases, the final plan is returned to the user’s treatment planning system for dose recalculation, review, and any necessary modification prior to clinical use. The RPA is intended to enhance clinical workflows while maintaining full clinician oversight; final plan evaluation and quality assurance remain the responsibility of the treating institution.
Algorithms
The RPA is powered by advanced artificial intelligence to automate data preparation, contouring, and key planning steps. Plan optimization and dose calculation are performed within the Eclipse Treatment Planning System (Varian Medical Systems), ensuring clinically robust plan generation. Dedicated, purpose-built algorithms perform multiple quality assurance checks, automatically flagging potentially suspicious contours or plans for user review. Final contours and treatment plans are returned to the user’s own treatment planning system for review, editing, and dose recalculation prior to clinical use.
Expected Cost
We are investigating how to make the RPA Program financially self-supporting, without making it unaffordable to clinics in low- and middle-income countries. Our goal is to make the RPA available to these clinics at minimal or zero cost.
User Registration
The registration process is currently only open to select collaborators. In order to ensure that clinics are able to use the RPA in their own clinical workflows, the registration process includes independent audits of the radiation outputs (all photon beams), and an independent audit of the local treatment planning system. Currently these audits are offered free to our collaborators.
